Reading Questionnaire
In preparation for your child’s first reading session, we ask you to complete our Reading Questionnaire. Completing this form will allow us to understand your child’s strengths and needs.

Technology Consent
Heart of Synergy uses non-invasive biometric technology (ear clip sensor or finger clip sensor) to monitor students' heart rhythm patterns, identifying in-the-moment heart coherence. For students 18 years and younger, we ask for parental consent to use biometric technology. Please review, print, and sign the consent form before the first session.
